If you have been scheduled to see Dr. Gross, you will be required to complete a new patient packet which contains all the necessary paperwork for your initial visit with us.

How to download and pre fill on the computer:
- Click the form
- A dialogue box will pop up, click the save button (you can either download as a Microsoft word or a PDF file to input your data)
- Then input your data, save it, and send it to our general email at the link below.
You can also bring them with you to your appointment or email them to us at general@carolinamacular.com
Please remember to bring these forms with you as well as your insurance card(s) and picture identification to your scheduled appointment.
If you are unable to download the forms online, please call our office at (843) 724-3456 and we will happily mail the forms to your home address.
